2. If needed, watch the tutorial video.

3. Create an account by clicking the “register” link to start an account. Provide a valid email address & password.

Note: It is important to include a valid email address. Email verification is required prior to registration. If you do not receive a verification email right away, simply email support@home-

4. Once you create an account in your email, you will need to verify your account by clicking on the link.

5. Now login at using the username & password you created.

6. Select the “New Clearance” button (upper left corner) to get started, then the year, your school, and the sport.

Note: You may upload your completed physical or submit in-person to the school athletic department.

7. Complete any required fields for student information, educational history, medical history and consent.

8. Press submit once you have completed the online clearance process.

9.  Once your clearance has been reviewed and approved, you will receive a confirmation email. Print and sign the confirmation page. 

10. Bring the signed confirmation page and a copy of the physical to our Athletic Trainer, Brady Rice. You may also leave it the the box in the front office for Brady. 

11. Once the signed confirmation page and copy of the physical are turned in, Brady will update the system and mark the student-athlete cleared for sports.


Important: Before being cleared for sports, the student-athlete needs to bring two documents to Brady Rice:

  • the online clearance (signed not initialed) confirmation page  
  • a copy of a current physical (stamped and dated) by a doctor, click here for physical form.



1. All incoming freshman ending 8th grade under a 2.0 will be placed on probation per CIFSDS.

2. During each grading period, students must attain a 2.0 grade point average in all enrolled courses on a 4.0 scale.

3. Further, student athletes must make progress toward graduation each trimester. If you are taking three classes, then you must pass at least two of the courses. If you are taking four or five classes, then you must pass at least three of the courses.

4. For the purpose of determining grade point average, all incomplete grades shall be computed as F’s until the incomplete is changed to a letter grade. At that point, grade point average will be recalculated using the grade(s) earned.

5. Accounting for eligibility will be the day after school wide distribution of grades. The grades that will be counted for eligibility are six week grades and trimester grades.

6. If students do not attain a 2.0 grade point average in the previous grading period, they will be placed on probation for the following grading period. Student athletes are only allowed one probationary period during their 9th grade and one additional probationary period during their combined 10th, 11th, and 12th grade years.

7. If students do not bring their grades up to a 2.0 grade point average at the end of the probationary period, they will be ineligible to participate for the following grading period.
